本标准规定了养老机构等级划分与标志、申请等级评定应满足的基本要求与条件、等级评定。本标准适用于养老机构等级划分与评定工作。
2规范性引用文件
下列文件对于本文件的应用是必不可少的。凡是注日期的引用文件，仅注日期的版本适用于本文件。凡是不注日期的引用文件，其最新版本（包括所有的修改单）适用于本文件。

3术语和定义
下列术语和定义适用于本文件。
3.1等级rank
在综合考察养老机构的环境、设施设备、运营管理、服务的基础上所作的分级。
3.2活动场所activity area
为满足老年人文化娱乐、健身活动需求而设置的室内和室外区域。
3.3责任事故liability accident
在养老机构的设施、场地内，因管理失职或操作不当，导致严重人身伤害及财产损失的事故。注：包括但不限于侮辱、虐待、走失、食物中毒、火灾。
3.4特困老年人elderly living in extreme difficulties
无劳动能力，无生活来源，无法定赡养、抚养、扶养义务人或者其法定义务人无履行义务能力的60周岁以上老年人。
3.5机构入住率occupancy rate
入住老年人总数与养老机构内总床位数的比率（%）。
3.6委托服务authorized service
受老年人及相关第三方办理托付事项的活动。
3.7康复服务rehabilitation service
采用科学方法、设施和设备，消除或减轻老年人身心、社会功能障碍，达到和保持老年人生理、智力、 精神和社会功能的活动。
3.8教育服务education service
为满足老年人身心发展需求，有计划地面向老年人开展知识、技能、思想、信息传递的一系列教育活 动与过程。
3.9居家上门服务home care services
为居家的老年人提供生活照料、膳食、清洁卫生、心理/精神支持等方面服务的活动。 4等级划分与标志
4.1等级划分
养老机构的评定分为五个等级，从低到高依次为一级、二级、三级、四级、五级。级数越高，表示养老 机构在环境、设施设备、运营管理、服务方面的综合能力越强。
4.2等级标志
4.2.1等级标志由五角星图案构成，用一颗五角星表示一级，两颗五角星表示二级，三颗五角星表示三 级，四颗五角星表示四级，五颗五角星表示五级。
4.2.2等级标志实行统一管理。
4.2.3等级标志的有效期为三年（自颁发证书之日起计算），到期应向评定机构申请复核。
5申请等级评定应满足的基本要求与条件
5.1养老机构应具备以下有效执业证明：
a）具有《营业执照》或《事业单位法人证书》或《民办非企业单位登记证书》；
b）具有消防安全合格证明；
c）具有房产证明或租赁使用证明；
d）养老机构内设的餐饮服务机构具有食品经营许可证；
e）养老机构内设的医疗机构具有医疗机构执业许可证或医疗机构执业备案证明；
f ）养老机构使用的特种设备，具有特种设备使用登记证；
g）养老机构提供其他须经许可的服务，具有相应资质。
5.2养老机构工作人员应具备相应要求或资质：
a）养老机构院长、副院长具有初中及以上文化程度；
b）养老护理员经岗前培训合格后上岗；
c）医生持有医师资格证书和医师执业证书，护士持有护士执业证书，特种设备管理人员具备相应上岗资质，其他专业技术人员持有与岗位相适应的专业资格证书或执业证书；
d）所有提供生活照料、膳食、医疗护理服务的工作人员均持有健康证明。 5.3养老机构的空间配置应满足以下要求：
a）老年人居室内床位平均可使用面积不应低于6m2，单人居室使用面积不低于10m2； b）老年人居室、卫生间、洗浴空间设置紧急呼叫装置或为老年人配备可穿戴紧急呼叫设备；
c）养老机构主要出入口、门厅、走廊、居室无障碍，能够满足轮椅、担架通行的需求，卫生间、洗浴空间无障碍，能够满足轮椅通行的需求；
d）养老机构出入口、就餐空间、活动场所安装视频监控系统；
e）公共区域和老年人居室应整洁，地面干燥，物品摆放安全合理，空气无异味；
f）卫生间设置便器、洗手盆，配有安全防护措施、无障碍设施，通风良好，空气无异味；g）洗浴空间有包括但不限于防滑地面、安全扶手等安全防护措施，并留有助浴空间；h）室内活动场所明亮、配置活动用品；
i）就餐空间桌椅牢固稳定；j)设接待空间；
k）公共区域设有明显的公共信息图形标志，并符合GB/T10001.1和GB/T10001.9的规定。
5.4养老机构的运营管理与服务应满足以下要求：
a）1年内无责任事故发生；
b）按照GB/T35796—2017中6.1.1的要求建立基本管理制度，包括但不限于行政办公制度、人力资源管理制度、服务管理制度、财务管理制度、安全管理制度、后勤管理制度、评价与改进制度；
c）按照MZ/T032—2012中第4章的规定建立安全管理体系；
d）对机构内各项管理制度、安全记录建档并留存；
e）服务提供与质量符合GB/T35796—2017中第5章的规定；
f）按照老年人的能力进行入院评估，制定照护计划；
g）与所有入住的老年人签订服务协议（特困老年人送养协议或社会老年人服务协议）；
h）建立老年人入住档案和健康档案；
i）组织老年人开展健康体检，每年不少于1次；
j）入住老年人Ⅱ度及以上压疮新发生率低于5%；k）每日至少组织2次适宜老年人的活动。
5.5一级养老机构应同时满足以下条件：
a）机构入住率不低于30%；
b）提供的服务项目包括但不限于出入院服务、生活照料服务、膳食服务、清洁卫生服务、洗涤服务、医疗护理服务、文化娱乐服务、心理/精神支持服务、安宁服务、委托服务。
5.6二级养老机构应同时满足以下条件：
a）机构入住率不低于35%；
b）提供的服务项目包括但不限于出入院服务、生活照料服务、膳食服务、清洁卫生服务、洗涤服务、医疗护理服务、文化娱乐服务、心理/精神支持服务、安宁服务、委托服务。
5.7三级养老机构应同时满足以下条件：
a）机构入住率不低于40%；
b）提供的服务项目包括但不限于出入院服务、生活照料服务、膳食服务、清洁卫生服务、洗涤服务、医疗护理服务、文化娱乐服务、心理/精神支持服务、安宁服务、委托服务、康复服务；
c）院长或副院长具有高中及以上文化程度；
d）至少有1名社会工作者指导开展社会工作服务；
e）设卫生间的老年人居室占能力完好、轻度失能、中度失能老年人居室总数的比例不低于50%，其中能力完好、轻度失能、中度失能老年人应符合MZ/T039的有关要求。
5.8四级养老机构应同时满足以下条件：
a）机构入住率不低于45%；
b）提供的服务项目包括但不限于出入院服务、生活照料服务、膳食服务、清洁卫生服务、洗涤服务、医疗护理服务、文化娱乐服务、心理/精神支持服务、安宁服务、委托服务、康复服务、教育服务；
c）院长或副院长具有大专及以上文化程度；
d）每200名老年人（不足200名的按200名计算）至少配有1名专职社会工作者；
e）设卫生间的老年人居室占能力完好、轻度失能、中度失能老年人居室总数的比例不低于60%；
f）每间中度失能老年人居室的床位数不多于4张，每间重度失能老年人居室的床位数不多于
6张，其中重度失能老年人应符合MZ/T039的有关要求。
5.9五级养老机构应同时满足以下条件：
a）机构入住率不低于50%；
b）提供的服务项目包括但不限于出入院服务、生活照料服务、膳食服务、清洁卫生服务、洗涤服务、医疗护理服务、文化娱乐服务、心理/精神支持服务、安宁服务、委托服务、康复服务、教育服务、居家上门服务；
c）院长或副院长具有大专及以上文化程度；
d）每200名老年人（不足200名的按200名计算）至少配有1名专职社会工作者；
e）设卫生间的老年人居室占能力完好、轻度失能、中度失能老年人居室总数的比例不低于80%；
f）每间中度失能老年人居室的床位数不多于4张，每间重度失能老年人居室的床位数不多于6张。
6 等级评定
6.1评定原则
全面客观，质量为重，注重实效，独立公正。
6.2评定方法
6.2.1等级评定总分为1000分，包括环境120分、设施设备130分、运营管理150分、服务600分。评分时按“优秀”“良好”“一般”“较差”打分并计算得分，评定内容与分值见附录A。
6.2.2养老机构评定得分不低于360分且每一分项得分不低于该项总分40%的，为一级养老机构；养老机构评定得分不低于450分且每一分项得分不低于该项总分50%的，为二级养老机构；养老机构评定得分不低于570分且每一分项得分不低于该项总分60%的，为三级养老机构；养老机构评定得分不低于780分且每一分项得分不低于该项总分80%的，为四级养老机构；养老机构评定得分不低于900分且每一分项得分不低于该项总分90%的，为五级养老机构。
6.3评定人员
6.3.1熟悉有关法律和政策，熟悉养老服务工作，并经等级评定培训合格。6.3.2具有维护评定工作客观、公平、公正的职业道德与操守。
6.3.3参与等级评定工作之前应向有关方面申明利益相关性。
6.4评定
6.4.1按第5章的要求，养老机构可根据自身情况向评定机构提出申请，并提交自评报告及相关材料。
6.4.2评定机构根据细则开展评定，并出具评定报告。
